Occupational exposure to pesticides causing Acute Myeloid Leukemia and death to a golf course superintendent. Bedsores to resident of a nursing home due to inadequate supervising and treatment. Loss of a testicle in an adolescent due to delay in diagnosis of testicular torsion.  Delayed diagnosis of a colon perforation during gallbladder surgery.  Delayed diagnosis of colon perforation during bariatric surgery. Misinterpretation of an x-ray of child’s right elbow fracture resulting in delay in treatment and misalignment of the child’s arm. Defective knee implant in an adult male resulting in early failure of the implant and revision surgery.  Improper manipulation of a patient’s leg following knee surgery resulting in a re-fracture of the knee requiring revision surgery and eventual loss of use of the leg. Malfunction of an escalator resulting in traumatic injuries to multiple passengers. Failure to employ proper fall prevention for a patient at a hospital, resulting in the patient falling in the bath room and fracturing his ankle.  Traumatic injuries to an airline passenger at an airport who was injured when a jetway collapsed due to inadequate maintenance of the jetway. Traumatic injuries and facial scars to a child due to inadequate assembly of a bicycle kickstand.  Unnecessary performance of an ERCP procedure by a gastroenterologist resulting in pancreatitis and death of a female patient.
